Once you receive a quote or invoice for test panel(s) requested, please make online payment. A non-refundable service charge (up to $100.00) applies if test is cancelled prior to the collection of sample(s). A higher service charge (including up to total cost for test) will be assessed if test is cancelled once the laboratory has received the collected sample(s).
